man qmail-spamthrottle
man qmail-spamt
mail qmail-newst

If you just want some reasonable defaults, do this:

echo "::1501:120000::1000::::" > /var/qmail/control/spamt
echo "." >> /var/qmail/control/spamt
/var/qmail/bin/qmail-newst
